    Dear Rapid City Medical Center patients,

    We have some important updates we want to make sure you have ahead of time.

    Rapid City Medical Center and Monument Health are coming together, and effective May 4, some of your providers and services will be moving to new locations. Many will remain exactly where you expect them. For those that are moving, we want to make sure you know exactly where to go before your next appointment.

    Please review the list below to find your provider’s new location, address and phone number. If you have questions, call 605-342-3280 or message your provider via the patient portal before May 1.

    We are grateful for the trust you place in us, and we are committed to making this transition as seamless as possible for you and your family.
