You can speed up the registration process by completing the Patient Registration form online before you arrive for your appointment. A link to the registration can be found at the bottom of this page. Please enter the information requested as completely and as accurately as possible.
Please remember that upon your arrival at Rapid City Medical Center you will be asked to verify your registration information; a copy of your insurance card and photo I.D. will be made.
